What is WALC?

The goal of the WALC Program is to engage, empower and activate Medicaid and Medicaid-eligible residents of the South End, West Hill and Arbor Hill (and nearby neighborhoods) to make consistent use of the preventative health care and wellness systems and to advocate with the health care system to promote integrated, culturally and linguistically competent accessible primary medical care access.

This brand new facility will host an interdisciplinary team of Trinity’s Resident Outreach Workers, Community Health Workers, students from the Albany College of Pharmacy and Health Sciences, and paid interns dedicated to connecting community members to health and wellness services. Additional health and wellness partners include Radix Ecological Sustainability Center and AVillage…, Inc.

Refugee Community Health Partnership Program (RCHPP)15 Trinity Place, Albany, NY 12202

We collaborate with refugee community leaders, volunteers, and community partners to organize events and services tailored to the special needs of refugees; provide individual and family mentoring to enable access to the health care system; and, at weekly drop-in sessions, provide assistance on a variety of health and wellness issues. 

Koinonia Primary Care, 533 Clinton Avenue, Albany, NY 12206

In addition to providing primary medical care, Community Health Workers will now be present in this office to facilitate and assist in keeping patients engaged in their health care. Community Health Workers will also help to re-engage patients who may be facing barriers to attending or following-up with their primary care appointments.

WALC is a partnership between Trinity Alliance of the Capital Region, A...Village, Inc., and the Radix Ecological Sustainability Center. Our focus will be on health insurance assistance, health education, understanding medications, transportation assistance, food & exercise, advocacy with health providers, and Trinity’s wrap around services to assist the whole person.
